MALE SURGICAL SERVICES:

CIRCUMCISION

The procedure is discussed before in detail with you and you can decide which type of result you have in mind. I perform a full classical circumcision, a partial circumcision, a frenuloplasty or preputioplasty or combinations of these procedures. I further perform all the custom -made variations of the procedures. Most of the time it really comes down to a combination of fulfilling your personal expectation and offering you the best practice advice depending on your particular case and requirement or clinical presentation. I will offer you best advice considering your personal preferences and considering all the options you will be offered the opportunity to make your final informed decision.

You will be given an opportunity to appraise the consent form prior to the procedure and you are under absolutely no obligation to continue with the procedure. You will further be given an opportunity to finally discuss the procedure, consent form and any of your questions before the procedure commences.

The procedure is performed in a standard methodology and aftercare will be explained and instructed. I perform the circumcisions in one of our theaters in the Clinic in Malahide. The procedure takes about 1 hour including the preparation and pre-med local anaesthetic applications. It will only be myself in the theatre and nobody else is allowed. I have a male doctor on standby in the Clinic to assist me if necessary.

I use laser for the surgery and as a result you would experience a great improvement in recovery time as a result of much less tissue damage and bruising to the tissue. The tissue healing is much improved and you might experience far less swelling in comparison to the standard surgical methods. The sutures are all dissolvable and need not be removed unless otherwise instructed. I would be advising you to take the next day off work to recover. Recovery usually will be 2-4 days, by which time most of our Clients will be able to return to work. You can expect the normal bruising and swelling that is associated with the procedure. Pain is usually well controlled with ordinary painkillers if needed. However, full recovery and wound healing will take about 21 days.

Wound inspection post-op is not necessary and you do not have to return for a follow-up unless you prefer to do so.

VASECTOMY

I perform the vasectomy in the standard way in one of our theaters using laser technology to perform a scalpel less procedure. I will offer you best advice considering your personal preferences and considering all the options you will be offered the opportunity to make your final informed decision.

You will be given an opportunity to appraise the consent form prior to the procedure and you are under absolutely no obligation to continue with the procedure. You will further be given an opportunity to finally discuss the procedure, consent form and any of your questions before the procedure commences.

The procedure is performed in a standard methodology and aftercare will be explained and instructed. The procedure takes about 1 hour including the preparation and pre-med local anesthetic applications. It will only be myself in the theatre and nobody else is allowed. I have a male doctor on standby in the Clinic to assist me if necessary.

1. double ligature of the tube at two places about 0,5 cm apart.

2. excision biopsy of the tube between the 2 ligatures - this goes to the pathology lab and

3. enveloping the ends of the tubes in the local tissue (forming a "pillowcase" for each end like a

blindfold) to keep them permanently apart.

Note: I perform the procedure in a way to allow for a reverse procedure by microsurgery in the future if this is of concern.

I would be advising you to take the next day off work to recover. Recovery usually will be 2-4 days. You can expect the normal bruising and swelling that is associated with the procedure. Pain is usually well controlled with ordinary painkillers if needed. I will issue a medical certificate on your request for recovery time.

Wound inspection post-op is not necessary and you do not have to return for a follow-up unless you prefer to do so. The dissolvable sutures do not need any after-care.

The cost for the procedure also INCLUDES cost for all pathology laboratory examinations for the tissue samples taken. This pathology test report is available to you after our procedure to confirm the Vas Deference (spermatic tubes) on both sides.

You will find clear instructions for the sperm analysis to be performed 3 months after your procedure on the day of your procedure. This is to confirm an infertile ejaculate. The cost of this procedure does not include the cost of this post procedure test. NB - You will still be fertile after the procedure and need to take precaution until such time that your infertility has been confirmed by sperm analysis. There would still be a lot of active sperm in your Uro-genital system after the procedure and you will still be considered fertile when you leave the clinic. It is only over a period of time and after about 3 months and after about at least 30 ejaculations that you would be ready for your infertility sperm analysis.

As a general rule you need to be aware that the color, look, smell, volume and consistency of your ejaculate will change gradually after the procedure. You will still ejaculate as before, but the ejaculate will be a clear - pre-cum like substance in a smaller amount that you would be used to. As a general rule we accept that a vasectomy will not change your libido or the quality of your sexual experience. In many cases this is improved as a result of a reduction in anxiety over the possibility of a pregancy.

You are welcome to request a light sedation anesthetic if you so prefer at no extra cost. This sedation anesthetic is generally used for short procedures and is a great way to reduce the discomfort associated with administering the local anesthetic. If you would like to avail of the sedation anesthetic you need to be 3 HOURS FASTING before the procedure.

MALE - ERECTILE DYSFUNCTION - IMPOTENCE

Erectile dysfunction symptoms and signs, like poor quality erection, loss of erection and general loss of libido is often caused by general impairment of blood flow and filling of the cavernous body (sponge like tissue) of the penis with blood when an erection develops. Aging is a general cause for the cavernous body to loose its function and for the loss of pressure a nd filling of the cavernous body.

HOW IT WORKS:

Carboxytherapy injections are a way to stimulate your own body, using what is naturally in your body, to promote an organic healing response to improve and rejuvenate tissue and skin quality. As a result of ageing, subcutaneous capillaries become dormant resulting in a decrease of circulation over time, depriving cells of oxygen; this, combined with environmental stresses and other variables, inhibits the ability of cells to regenerate and function properly. In the penis Carboxytherapy stimulates the release of growth hormone in the tissue of the penis and triggers a cascade of hormones and tissue rejuvenation factors to rejuvenate and regrow the tissue of the cavernous body (sponge tissue that expands during erection). The treatment is completely natural. Some people may experience some mild discomfort, however, most feel a mild tingly sensation which then warms. The Carboxytherapy System provides heated gas, which dramatically reduces discomfort.

Some clients prefers to avail of a short acting sedative for the procedure. The sedative works around 20 - 30 minutes. If you consider this allow at least 90 minutes for your visit to allow you to fully recover before leaving the Clinic. There is no additional charge for the sedation.

This is performed in one of our theatres in our Clinic. You will be provided with a prescriptiom to collect the device at your own cost from a Pharmacy - if you call our Clinic we can fax a prescription for you to your own Pharmacy. Please bring your device along to the appointment. Best practice and advice is to book your appointment for the procedure on day 1,2 or 3 of yoru normal cycle - the cervix is dilated in this period and the procedure is much more comfortable performed with the cervix dilated. If you are not sure about predicted dates you can call our Clinic on your first day of your period and our staff will try to accommodate you on any of these days.

FEMALE - VAGINAL AGEING / DYSFUNCTION

Vaginal Rejuvenation through Carboxy therapy

What is vaginal rejuvenation, how does it work and what can be expected.

In our practice we started doing a protocol for vaginal rejuvenation about 18 months ago. It entails insufllation of the labia / inner lining of the vagina with Carbon Dioxide, at strategic points at a specific flow rate and volume.

At the same time we started doing treatments for males for Erectile Dysfunction and we recording great results for managing the same. This treatment option for males is a great long term solution for erectile dysfunction instead of taking the likes of Viagra and Cialis on a regular base. The treatment offers a new lease of life for males suffering from erectile dysfunction and we report similar results for females suffering from Vaginal ageing.

Many of the patients had watched 90210 on TV and the surgical option of vaginal rejuvenation. It had appealed to them to make them feel more confident and regain some youth. The problem was that after the surgery, while the tissues were tighter, which was a better sensation for their partner, they felt that sex had become more painful, and that other facets like vaginal dryness, and poor libido or loss of sensation could not be treated surgically.

We now do treatments on women daily, and the results have been reasonably consistent - improved sensation, much improved lubrication - to the point that even postmenopausal women do not need KY or other external lubricant, the most remarkable effect has been an improvement in libido in the women. This was an unexpected though very welcome side effect. Many of the women that have come in, even as young as early 30's have stated that after the birth of their babies, their libido or sexual drive just disappeared.

There are many factors that can cause this - the sleep deprivation and the production of a hormone called Prolactin affects sexual drive, but for many of these women their sex drive never recovered and they are left with guilt and embarrassment and eventually failed relationships with no real help available to remedy the problem. Testosterone supplements can help but have their own set of side effects which are unacceptable.

So when we speak of Vaginal Rejuvenation, we are speaking of regaining function as well as a cosmetic improvement. Function includes intensity of sensation, lubrication, response and climax time, as well as improvement in libido.

How does it work : Carbon dioxide injection lowers the oxygen pressure in the tissues, this has to be enough though to reach a therapeutic effect. If sufficient amounts are infused, then a growth factor is released that causes new capillaries to grow into the area and be regenerated - around the nerves, improving sensation and response, around the glands improving lubrication, and in the muscle and soft tissues, improving tissue density and thickness.

What can you expect : The treatment entails the injection of carbon dioxide at specific points, it takes around 10 minutes to do. The tissues feel slightly tender or puffed out for at best 30 minutes or at worst a day, almost like period pains, but this is not severe, and thus far no clients have stopped treatment due to discomfort levels. The needle is really tiny and the procedure is very simple.

By the 4th treatment we usually ask for feedback - we usually do the first 4 on a trial basis, and then the client does maintenance treatments once a month. The feedback has included the following and more :

better orgasm and reached much faster

cosmetic plumping of tissues

quicker bodily response when the partner initiates sexual contact
